Steps to receive and view your 2015 1098-T

Access your 1098-T tax form online through your myFSU account:

1. Log on to my.fsu.edu with your username and password
2. Click "Student Financials" in the left-hand navigation menu
3. Select the "Tax Information/1098-T" link
4. You will be sent to the 1098-T page in your Student Center
5. Click the tax year and a PDF of your 1098-T will open

Parents / Delegates should complete the following to access their student's 1098-T tax form online:

1. Log on to my.fsu.edu with your username and password
2. Click "1098-T Tax Form" under the "Financials" section
3. You will be sent to the 1098-T page
4. Click the tax year and a PDF of your student's 1098-T will open

Steps to receive and view your 1098-T from previous years

1. Log on to my.fsu.edu with your username and password
2. Click "Student Financials" in the left-hand navigation menu
3. Select the "Historic Tax Information" link
4. You will enter the ECSI website
5. Under “Account Tools” select the "Tax Information" link
6. Click the tax year you would like to view

Parents / Delegates (or anyone encountering trouble accessing the 1098T through myFSU) can log into the ECSI site directly.

1. Navigate to: http://ecsi.net/taxinfo.html
2. Click the "click here to retrieve your logon credentials" link and follow the instructions provided by ECSI. If you need assistance with this process, please contact ECSI's 1098-T support line at 866-428-1098.

Please note that the 1098-T is available online by February 1st of each year. Students will receive an email notice when the form is available.

W-9S

What is the W-9S form?

This form is used to determine a student's correct identification number. This could be a Social Security Number (SSN) or an Individual Taxpayer Identification Number (ITIN). To provide accurate information on your 1098-T Florida State University requires your correct identification number. If you do not have one of the aforementioned identification numbers, do one of the following:

  • To apply for an SSN, visit: http://www.ssa.gov/forms and follow the instructions on the Social Security Card application.
  • To apply for an ITIN, visit: www.irs.gov and search for the W-7 form. Follow the instructions listed on the form.

Only complete and submit a W-9S form if you have been contacted by the University and were instructed to complete one.

 

How do I complete the W-9S?

Complete Parts I and II of the form. This means providing the following:

  • your name
  • your taxpayer ID number
  • your address
  • your city, state, and zip code
  • your signature and the date of signature

Once you have completed the form please submit it to the Registrar's Office in person. The Registrar's Office is located on the third floor of University Center A, room A3900.
